Go to www.swol123.net and click REQUEST TO JOIN.

The code is DRAGONS.

I will get an email request which I will approve as soon as I see it.

Once approved, you need to click the MY INFO tab on the left side of the home page.

This will bring up a series of more tabs (general info, address, emergency contact info, insurance, etc).   ALL OF THIS MUST BE COMPLETED. Omit the SSN

To upload the physical:

Go to the home screen and click FORMS.

Then click ADD. (Add all 4 pages of the physical)

Select the file the physical is saved under.

Click OK.

CLICK SAVE ON THE HOME PAGE BEFORE YOU LEAVE SPORTSWARE.
